dr. kenzō tenma young japanese brain surgeon, working @ eisler memorial hospital in düsseldorf. tenma dissatisfied political bias of hospital in treating patients, , seizes chance change things after massacre brings fraternal twins johan , anna liebert hospital. johan has gunshot wound head, , anna mutters killing; tenma operates on johan instead of mayor, arrived later. johan saved, mayor roedecker dies; tenma loses social standing. director heinemann , other doctors in tenma s way mysteriously murdered, , both children disappear hospital. police suspect tenma, have no evidence , can question him.


nine years later, tenma chief of surgery @ eisler memorial. after saving criminal named adolf junkers, junkers mutters monster. tenma returns clock junkers, finds guard in front of junkers room dead , junkers gone. following trail construction site of half-finished building near hospital, tenma finds junkers held @ gunpoint. junkers warns him against coming closer , pleads him run away. tenma refuses, , man holding gun revealed johan liebert. despite tenma s attempts reason him, johan shoots junkers. telling tenma never kill man saved life, walks off night, tenma shocked stop him.


tenma suspected police, particularly bka inspector lunge, , tries find more information johan. discovers boy s sister living happy life adopted daughter; traces of terrible past few nightmares. tenma finds anna, subsequently named nina foster parents, on birthday; keeps johan, late stop him murdering foster parents. tenma learns origins of monster : former east germany s attempt use secret orphanage known 511 kinderheim create perfect soldiers through psychological reprogramming, author of children s books used in eugenics experiment in former czechoslovakia. tenma learns scope of atrocities committed monster , , vows fix mistake made saving johan s life.
