Clinical significance Buccal space

a hematoma may create buccal space, e.g. due hemorrhage following wisdom teeth surgery. buccal space abscesses typically cause facial swelling on cheek may extend zygomatic arch above inferior border of mandible below, , anterior border masseter muscle posteriorly angle of mouth anteriorly. unless space involved, tissues around eye not swollen. treated surgical incision , drainage, , incision located inside mouth avoid scar on face. incision placed below parotid papilla avoid damage duct, , forceps used divide buccinator , insert surgical drain buccal space. drain kept in place variable period of time following procedure.


long standing buccal abscesses tend spontaneously drain via cutaneous sinus @ inferior of space, near inferior border of mandible , angle of mouth. untreated cutaneous sinus can cause disfiguring soft tissue fibrosis, , tract can become epithelial lined.


odontogenic infections

an abscess originating tooth has spread involve buccal space. above, deformation of cheek on second day. below, deformation on third day.


sometimes buccal space reported commonly involved fascial space dental abscesses, although other sources report submandibular space. infections originating in either maxillary or mandibular teeth can spread buccal space, maxillary molars (most commonly) , premolars or mandibular premolars. odontogenic infections erode through buccal cortical plate of mandible or maxilla either spread buccal vestibule (sulcus) , drain intra-orally, or buccal space, depending upon level of perforation in relation attachment of buccinator maxilla above , mandible below (see diagrams). infection spreads in both directions buccinator partial barrier. infections associated mandibular teeth apices @ level inferior attachment, , maxillary teeth apices @ level superior attachment more drain buccal space.
